Q. Do you accept my insurance?
The best way to determine if the clinic accepts your insurance is to:
Review the provider directory provided by your insurance company, which should list hospitals and physicians covered by your insurance
Call the telephone number on the back of your insurance card and ask your representative if the physician, provider, or facility is participating
Visit your insurance company's Web site
      Or, call us and ask.
Q. When can I expect to receive a bill? Why was I sent a statement when my insurance company is supposed to pay my bill?
For patients with health insurance: once your insurance company has been billed and has responded to us, we determine how much you may owe and bill you. You may receive a statement showing that your insurance company has not paid the bill. Whether you have insurance coverage or not, you as the patient are ultimately responsible to make sure your bill is paid. If you receive a statement showing that your insurance company has not paid, it may be helpful for you to contact your insurance company to ask why payment has not been made.
Q. How do I pay my bill?
Payment for your portion of the bill (co-insurance, co-payment, deductible, uncovered services, full bill) is due prior to the time services are provided or within 30 days of receiving a bill. To pay your bill online or set up a payment plan, click here.  Payment can be sent by check, money order, or by credit card. Please do not send cash through the mail. If you choose to pay in cash, please pay in person. Please include the patient's name and account number with your payment.
Q. Why are you asking for my deductible, co-insurance or co-payment at the time of my visit?
If you have a co-payment, deductible, and/or co-insurance with your insurance company, that amount is due at the time of your visit. This is part of the contract you have with your insurance company. Paying this at the time services are rendered not only keeps you compliant with your insurance contract, it also helps our office keep the billing costs down.
Q. I made a payment on my account, yet I received a collection notice. Why?
In order to prevent collection notices from being sent, you must pay 33% of the total patient balance. If you are not able to pay 33%, please call one of our certified Account Specialists, so that we can assist you.
Q. What if I can't afford to pay my bill?
We accept a wide variety of payment methods and will offer resources to assist the patient or responsible party in resolving any outstanding balance. To setup a payment plan, or to make a payment online, click here. We have a financial assistance program you can apply for as well. To view our financial policy, click here.
Q. Why didn't my insurance pay, or only make a partial payment?
You should have received an Explanation of Medical Benefits (EOB) or Explanation of Payment (EOP) from your insurance company, showing how they considered your claim. This EOB/EOP should have a contact telephone number or web site where you can reach your insurance company for questions. Please contact your insurance company or benefits office with questions about denied claims or charges. You may also be required to pay a co-payment, coinsurance, and/or a deductible for your healthcare expenses. Your bill may include charges that you are responsible to pay, such as non-covered items or out-of-pocket expenses.
Q. What if I don't have insurance?
Please contact one of our certified Account Specialists, who will be happy to discuss payment arrangements with you. Please call 800-491-0038 or 712-279-5830.
